I really feel like a "half dead fish" this morning, so tiring.... On call yesterday(last on call of the month..) & as usual, my "luck" hasn't been "good".. :( Feel so sleepy in the early morning today when the craniotomy & evacuation of EDH or extra dural haematoma case going on.. Finish around 5:15 or 5:30 am; can't remember the exact time... My mind telling me "I want to sleep... :P Well, I haven't 100% recover from my sickness yet.."
Ate something in the pantry together with my nurses early morning today.. The bekal they bring from home - white rice with petai & chilli paste(hot & spicy, so very nice.. kekke) , sambal tempoyak (tempoyak I eat a lot before this, but this is the first time I tasted sambal tempoyak :P), kerang & fried fish... & keropok. Quite a tasty meal...
I sleep the whole day today to compensate. Wake up for dinner just now & went back to slumber land again shortly after dinner. Now I feel better...

Cholesterol level
Since I take blood for blood investigations that day, I send my blood for Fasting Lipid Profile (FLP) to check my cholesterol level as well...
Wow.. I didn't expect my cholesterol level to be like that :P 3.5 mmol/l.
My previous cholesterol level is 3.9 mmol/l (which is quite good already), & now, it's even lower... Hahahaa...
Fasting Lipid Profile
Total Cholesterol 3.5 mmol/l (3.5 - 5.7 ; <5.7>6.7 high risk)
Triglycerides 1.0 mmol/l ( 0.6 -2.3 ; <1.7>2.1 high risk)
LDL-Cholesterol (bad cholesterol) 1.8 mmol/l (<3.3>4.9 high risk)
HDL-Cholesterol (good cholesterol) 1.2 mmol/l (0.9 - 1.7)
% HDL-Cholesterol 34% (>25 low risk, <15 high risk)
mmm.. my HDL cholesterol level come down compared to previously. My previous reading was 1.6 mmol/l. (Lack of exercise lately...) Must do more exercise to bring up the good cholesterol HDL...

Sick
Didn't update my blog for the past few days cos I'm sick..
On call last Saturday & it was a busy day for me.. Quite tiring actually & not enough rest .. :( Manage to rest around 3:30 am in the morning.. (Err.. Darren sent me an sms at 2:30 am telling me that he is taking the midnight express & now he was on the way to Penang). Aiyoh.. luckily I was awake & had case going on at that time, or else I will scold him back loh, for waking me up at this weird hour... kekke... This is the reason why he came to Penang :) Very touching story..
Just a side topic, hahha.. how come so many people like to sms me at weird hours one? :D Previously there's this Entreri sms me at 4:40 am telling me he sprained his ankle ; another time, kktan sms me at 5 am (I just went to sleep at that time (on call in ICU that day), so I didnt answer his sms, but walao.. he called me again later on at 5:30 am... ggrrrr.... )
The next day, I was down with URTI initially(cough, running nose & sorethroat), likely my colleague passed the germ to me :( ; Then followed by high grade fever with chills(up to the temperature of 39.1 celcius or 102.4 Farenheit), myalgia or muscle pain, retro orbital pain, malaise & lethargy.. Even no energy to walk. I check my blood for Full Blood Count/FBC (luckily platelet count normal, so unlikely to be Dengue Fever..) Too many Dengue Fever cases & Malaria cases around in the hospital for the past few days .
Drink so much water for the past few days.. Never in my life I drink as much water as what I have drink for the past few days.. Now, I'm recovering well :)

Needle Prick Injuries
Frankly speaking, I get needle pricks injuries few times already for the past few years. (I think most medical staffs experienced this needle prick injuries as well..) Not that I'm careless.. I try to be careful most of the times, but sometimes, mishap still happens & "accidents" are inevitable.. Few times, it occured when withdrawing drugs or after injecting the drugs through the drip line (which is still alright cos the needle is sterile). The most important thing after taking blood is don't recap back the needle..(cos this can easily cos needle prick injuries, and these needle are contaminated with patient's blood !!! ).
One "sinister" needle prick injuries I had was three years ago... After inserting the triple lumen central venous lines (CVL) & pulmonary artery catheter (PAC) for one lady patient inside the operating threatre; She was scheduled for mitral valve replacement (MVR) & aortic valve replacement (AVR).. I was stitching the CVL & PAC catheter to secure it, & somehow I accidentally prick myself with the sharp needle while stitching .. aiyh... Although I'm worried, but least not so bad, cos she had baseline investigations for HIV, Hep B, Hep C & VDRL prior to the open heart surgery.., which were negative (It's routine to do all these investigations over here for patients before undergo any open heart surgery).
The other friends of mine was not so lucky.. They had needle prick injuries to patients which were known to be Hepatitis B carrier, Hepatitis C carrier & even HIV patients !! Imagine the trauma that those health care personnels have to undergo after the needle prick injuries with known "contaminated blood" especially HIV. The immediate blood result although shown to be negative doesn't mean anything cos of the "window period".. Have to repeat the investigations few weeks later or even up to 6 months later to get the proper/final confirmation that one is free from the disease. This 6 months period is really the torturing & distressing period. Luckily my friends are "safe" from the diseases..
I had my HIV, Hep B & Hep C blood test done as well, & thank God nothing sinister happened... all the results were negative :)

BCDFE
Reading about inhalational agent today & come across this thing BCDFE :)
Well, it's the breakdown product of Halothane... (It would be more special if the name is BCDEF)
Degradation of Halothane in soda lime will give rise to vinyl compound, known as 2-Bromo, 2-Chloro, 1,1-DiFluoroEthane (also known as BCDFE)
at least this is easier to remember than breakdown product of Sevoflurane..
Degradation of Sevoflurane in soda lime will give rise to vinyl ether compound, also known as Compound A or Pentafluoro Isopropenyl Fluoromethyl Ether (or PIFE). *Pening* to remember this long "organic chemical" name..

HIV/AIDs
I had one HIV patient (ex IVDU drug addicts) underwent surgery in my operating theatre two days ago & he came back for another operation today. At least this patient is very honest, he informed us that he has HIV. Some patients keep the fact that they have HIV or AIDs, cos scare that they will be "discriminated " cos of their disease. The medical personnel (doctors, nurses, attendants etc) just have to take extra precaution (although we do practise universal precaution on everyone), when handling or dealing with blood taking or whatever procedures that come in touch with blood etc in these patients .. like setting branula, taking blood, or performing surgical procedures. If one accidentally get needle pricks injury from HIV patients, then it's really traumatised .. I have came across & treated many HIV or AIDS patients already throughout these years. It's not "scary" as long as one is alert & careful..
One of the problems with HIV patients(referring to those IVDU cases) is very difficult to get intravenous access. Basically most of their veins are either hardened or thrombosed cos of regular intravenous injections. Most of the time, the femoral vein is thrombosed as well.. If lucky, can still manage to set branula at the neck vein or external jugular vein,(provided these IVDU cases don't inject the drugs via the neck veins)

Reasons why alcohol is "good" for the human species
Get this joke from Dr Palani regarding alcohol :)
Sometimes when I reflect back on all the wine I drink I feel ashamed. Then I look into the glass and think about the workers in the vineyards and all of their hopes and dreams If I didn't drink this wine, they might be out of work and their dreams would be shattered. Then I say to myself, "It is better that I drink this wine and let their dreams come true than be selfish and worry about my liver."
"When I read about the evils of drinking, I gave up reading."
"24 hours in a day, 24 beers in a case. Coincidence? I think not."
"When we drink, we get drunk. When we get drunk,we fall asleep. When we fall asleep, we commit no sin. When we commit no sin, we go to heaven. So, let's all get drunk and go to heaven!"
Here's how it went:

Here's how it went:
"Well ya see, Norm, it's like this... A herd of buffalo can only move as fast as the slowest buffalo. And when the herd is hunted, it is the slowest & weakest ones at the back that are killed first This natural selection is good for the herd as a whole, because the general speed & health of the whole group keeps improving by the regular killing of the weakest members.
In much the same way, the human brain can only operate as fast as the slowest brain cells. Excessive intake of alcohol, as we know, kills brain cells. But naturally, it attacks the slowest and weakest brain cells first. In this way, regular consumption of beer eliminates the weaker brain cells, making the brain a faster and more efficient machine. That's why you always feel smarter after a few beers."

old TVB cantonese series

Went to meet Foong in Coffee Bean just now. I arrived a bit early, so I went for a walk inside Island Plaza. Stopped by Speedy Video Distributors to have a look, & end up buying few old HK TVB cantonese drama series VCD sets. I watched those series before previously, and in fact many years back.. Summer Kisses Winter Tears (by late Anita Mui) 1984 series; I watched the series (on video tapes) previously in 1985, primary school time. And another set The Challenge of Life (by Lau Ching Wan, Leon Lai etc) in late 80's, .. I think I watch that series in 1989 ... during my secondary school times. Hahaha... I seldom watch tv nowadays.. Bought it just for collection only :P
Met Foong in Coffee Bean later on.. I have Malibu Dream (Strawberry Flavour) & have chit chat with him.
Nasi Lemak 椰浆饭
This is the delicious "unhealthy" Malaysian food that I have for my breakfast today. Haven't eat nasi lemak(椰浆饭) for so long, I think at least for few months already. Simple but nice..Bought this inside the operating threatre pantry. One of our hospital attendants sold this every morning for extra side income :) Whatever food sold inside the operating threatre pantry or coffee room sure very "laku" one .. in other word, good business; cos the doctors(surgeons or anaesthesiologists/srub nurses/GA nurses or MAs can't go out for food...
Insomnia
Don't know why.. having insomnia on Saturday night.. Still quite wide awake even at 4:45 am in the early morning.. If I don't have to work the next day, then it's still alright, but... I'm working.. not only working, but in fact, I'm on call on Sunday.. I don't know I manage to fall asleep at what time later on, but it's just a "short sleep" cos I need to wake up at 6:45 am.. I think basically I slept for less than one & half hour... Feel so tired when I wake up on Sunday morning. Luckily my on call on Sunday was not so busy.. So, still manage to cope & get some rest in between, or else I will be "half dead" :P

Generic & Trade Name
I was on call in ICU that day... During my ICU night round, I ordered T Alprazolam 0.5 mg ON for one of my patients. Half way doing round, I got a SOS call from the cardio MO, asking for help cos of failed intubation, whom I attended to stat. (In fact, got called 3 times that day/night to help out for failed intubation.. Two of thoses cases, well, actually not difficult intubation, but the junior MO unable to intubate, just that the last one from NeuroSurgery was indeed difficult intubation, with Cormack & Lehane grade 3 laryngoscopy.)! By the time I reached back ICU, the night shift nurses took over already. The nurse who was in charge of the patient whom I prescribed T Alprazolam told me that T Alprazolam was not available & she can't indent the medicine because pharmacy was closed.
Me : Are u sure we don't have tab alprazolam?
Nurse : No, we don't.
Me : I thought we have tab alprazolam in our DDA cupboard? Alprazolam is one of the commonly used DDA drugs
Nurse : No, we don't have alprazolam in our DDA cupboard.
Me : Then, what are the sedative drugs we have in the DDA cupboard at the moment?
Nurse : We have Xanax, valium, lorazepam etc..
Me : Hah? Aiyoh... Xanax is Alprazolam lah... Alprazolam is the generic name for Xanax!!
Well, some of the nurses (including doctors as well) are not familiar or get confused with the name of the drugs cos most prescription drugs have different generic (official) names & trade names (also called (brand, or specialty names). And some drugs even have few trade names - different names for different manufacturers.. Confusing for those who are not familiar with the drugs. Most of the staffs here are familiar with the trade name Xanax but not the generic name, Alprazolam. (Although both are the same thing). Well, generic names are usually more complicated & harder to remember compared to trade names. Personally, I still prefer to use generic names than trade names.
Some of the commonly used sedative drugs/medicine from the benzodiazepine group;
Trade Name (vs) Generic Name
Dormicum; Midazolam
Valium; Diazepam
Xanax; Alprazolam
Ativan; Lorazepam
Another more important group of drugs is antibiotics. It is indeed hazardous if the wrong antibiotics is served or administered especially if the staff is not familiar with both the generic & trade name..
15% of all reported medication errors between 1995 & 2000 worldwide arise from some kind of name confusion.

Brunch
Chinglish
I receive this nice stuff via email from a close friend of mine.. This is really hilarious... even an Englishman could not construct sentences using numeric, which is exclusive only to Malaysians and Singaporeans.
Ah Beng was asked to make a sentence using 1,2,3,4,5,6,7,8,9 and 10. Not only did he do it 1 to 10, he did it again from 10 back to 1. This is what he came up with...
1 day I go 2 climb up a 3 outside a house to peep. But the couple saw me, so I panic and 4 down. The man rush out and wanted to 5 with me. I run until I fall 6 and throw up. So I go into 7 eleven and grab some 8 to throw at him. Then I took a 9 and try to stab him. 10 God he run away.
So, I put the 9 back and pay for the 8 and left 7 eleven. Next day, I call my boss and say I am 6. He said 5, tomorrow also no need to come back 4. He also asks me to climb a 3 and jump down. I don't understand, I am so nice 2 him but I don't know what he 1.

Retire at 65 years old?
The topics regarding all government doctors and health service personnel will enjoy more benefits, effective Jan 1 this year, under a move to retain them and attract new employees really attracts me... But when I reach the statement "For doctors, the incentives include their employment being extended to age 65 and tax-exempted allowances."
Aiyoh.. What the heck lah.. Extend the employment to the age of 65 years old? What? I want to retire at an earlier age in fact, & not to extend the service.. This doesn't sound attractive to me at all :D Why want to retire so late? I don't understand :)
Enjoy life lah..
(p/s : btw, I'm not "workaholic")

Old uni friend..
Receive a phone call from a friend, Dr Chia yesterday night... Can't meet up with her yesterday cos I was on call. She was working in KL. She is one year my junior previously during my undergraduate time. In fact I haven't meet her since my graduation in 1998, although we still keep in touch on and off via handphone. How time flies.. Went out for dinner together with her & her hubby tonight. Really nice meeting up with old friends :)
Tired...
On call 3 times this week .. Tiring.. Monday, Wednesday & next on call will be this coming Sunday.. Haven't really recovered from the tiredness from the previous on call, then have to on call again.. Damn tiring.. Only manage to go to sleep at around 4:45 am early morning today, & supposed to wake up at 6 am to do my morning round. But, since I'm too tired, I "overslept" a bit & woke up only around 6:40 am.. Cos of this, I have to write faster during my morning ward round in order to complete my rounds, cos so many patients to review ...(Tiring lah.. working the whole day from 8 am till 4 something in the morning the next day, some more I have unstable, critically ill patients to take care of ...) Anyway, although I wirte fast,my handwriting is still nice & readable :P

Worms...
I posted something about eating worms previously.. So far, I have tasted the sago worms & the silk worms..
Too bad I don't have digital camera earlier on when I ate the sago worms.. But I manage to get the photo of how the sago worms look like, with the courtesy of Fredy Can't remember how it tasted like already.. Click here to have a look how they "collect" the worms...
Well, that's not the only type of worm that I have tasted.. Later on I tasted the silk worms as well, during my trip to Beijing.. This is the snapshot I took from the WangFuJin night street in Beijing.
Grace from my hometown (Sarikei) mentioned before that she doesn't dare to eat the sago worms, but I salute her, cos she eats Cikada, which sounds more scary to me :P

Female Genital Mutilation (FGM)
Many knows about circumcision in male population, but guess not many have heard about female circumcision. I saw a case Female Genital Mutilation(FGM) in labour lately. FGM is practiced mainly in few countries like Africa & middle east etc..
She is a foreigner, from North Africa. The "female circumcision" practised there or FGM is a social custom, not a religious practice. In fact, this is the first time I see such a case after working for so many years... Her's is FGM type 3, involving excision of part or all of the external genitalia and stitching/narrowing of the vaginal opening (infibulation); - clitoridectomy , excision of the labia minora , & cutting of the labia majora which are then stitched together in order to form a cover over the vagina, with a small orifice left(which can only admit two fingers.. Difficult for her to undergo vaginal delivery cos there's a high possibility of severe perineal tear.. (well, have to cut off the fused part & restitch back again if going for vaginal delivery). She underwent Caesarean Section to deliver the baby..

TT in MVM
Thursday night, joining the TT session organised by Alex. Kenneth came to fetch me, followed by Darren. Meeting up with Alex in Mid Valley Mall at 7:30 pm. Enteri & Lcti arrived much later.. about half an hour later loh.. Long wait.
The place AC planned to bring us to for dinner was closed.. Too bad... So we proceed having dinner in the Food Court. Eating Nasi Kerabu for dinner. Aik.. Enteri didn't bring the Maggie Mee for me, but never mind lah.. eatin nasi kerabu is better than eating maggie mee, hahah... Hawks dropped by as well half an hour later to join in the chit chat session.. Enteri is indeed hilarious. Abuthen, Enteri, u made that "someone" sad leh.. mistaken the 20 year old boy as 30 years old liao :P

Meeting up with old friends
Wednesday night.. Time to meet up with my good old friends. They are my coursemates during my undergraduate time (whom we spent 5 years together previously), & my close friends as well... However, we seldom have time to meet each other since graduation. Even though most of them are working in KL, they hardly have gathering or meet each other as well. The last time we met each other was during my previous trip to KL, one year back in June 2004..
This time, I manage to meet 6 of them. Each of them specialises in different field/discipline. Dr Bee (Physician, planning to subspecialise in Haematology), Dr Ang (Paediatrician), Dr Lim (Obstetrician & Gynaecologist to be), Dr Yap (Anaesthetist to be), Dr Leong (Radiologist) & Sharon (Pharmacist). My other friend, Dr Sia (Surgeon, planning to subspecialise in Neurosurgery) couldn't made it cos he's not free..
So nice meeting back good old friends.. Had a good long chat with them.. We can basically talk about anything. So fast 7 years plus passed already, I mean since our graduation previously in 1998. Among my Chinese coursemates, there are paediatricians, physicians, surgeons, anaesthesiologists, obstetrician & gynaecologists, radiologists, orthopaedic surgeon, ophthalmologists etc.. Some came out & work as general practitioners (GPs) opening up own clinics or partnership with others. Some work in health clinics as well..
Half way talking with my friends, one Indian old man came to our table, selling lottery tickets. We told him we were not interested, since none of us indulged in this things. Exactly at that moment, my handphone vibrates (incoming SMS). So I bent down my head & put my hand inside my pocket to take out my handphone. When I lifted up my head.. alamak... I noticed so many pairs of eyes starring at me.. I was kinda shocked! Then only I realised.. they were thinking that I was taking out $$$ from my pocket to buy the lotteries, hahaha... Even the lottery man also thought I was interested.. cos I noticed he came next to me & swinging the lottery tickets infront of me :P Sorrylah, pakcik, I'm not buying :D In fact, I never buy any lottery before in my whole life leh...
Aiyoh.. sms from AC... hahah.. thinking about the "misunderstanding" & the "embarassment" he caused just now. Hahaha... I must tell him the incident one of these days :P

Wong Ah Wah - Dinner Gathering
Wednesday.. Had lunch together with 3 of my ex colleagues. They just left my working place not long ago, to be exact, end of March this year; but still there's lotsa things to talk about.. :)
Night time, had dinner gathering at Wong Ah Wah restaurant in Jalan Alor.. together with BabeKL & her hubby, Weezy, John, Will, Lcti, & Alex. This is the famous dinner hangout place for the forummers, & the grilled chicken wing or "siu kai yek" here is very popular :) However, this was the first time I had dinner here..
One funny thing that night, regarding Alex; He arrived a later than the others.. He drag a chair from the other table & sat down on our table. Somemore, he hold few VCDs/DVDs inside a plastic bag. Giving us a "shock", thinking that he was a VCD seller :P Hahaha... Poor Alex :P
Nice food :) We 'sapu' all the dishes in a short while. So many hilarious moments; really enjoy the gathering & the chit-chat. Thanks for the great time :)

Dinner in Station One, Cheras
Haven't attend any lectures for so long already.. The last time I attended the full day lecture is during my pre-clinical year (Year 1 & Year 2, which was way back in 1993 & 1994).. Aiyk.. So tired, and really can't concentrate after one or two hours.. Some more, the topics are not interesting & "dry"... about research & statistics... My mind wander some where far away during the second or third lecture already :)
Having lunch together with my ex colleague. She was still the same as before, with her cheerful personality :) Really had fun talking & recalling back the old days.. She was pregnant at the moment; her second baby :)
Tonight I had dinner in "Station One" in Cheras together with my KSL, Kenneth & Darren. Kenneth came to fetch me first then followed by fetching Darren. Aiyoh, we had to wait for this Darren "tai siu yeah" for 15 minutes inside the car... Wanna know why? Cos he "cooks porridge" with his darling pulak on the phone till forget about us waiting for him inside the car.. :P
(Darren, don't shoot me ah for posting this :P)

BKT(肉骨茶) Dinner in Klang
Went to Klang together with John to visit Will later on in the evening. Will brought us around Klang for some sight seeing,(including Port Klang as well, hehehe...) & then we had bak kut teh (肉骨茶) or BKT for dinner in Kee Heong Restaurant. BKT is the famous food in Klang & since Will likes BKT so much, (till someone mentioned he has BKT blood :P), so must try out how delicious or how special is this Klang BKT :)
Initially Will ordered the big claypot BKT, shared among 3 of us. The 2 guys were still hungry (growing boys mah..), so Will ordered another medium sized claypot BKT.. Seems like our table had the most BKT compared to the other tables.. *paiseh* , The next table sharing one medium sized claypot BKT among 4 people leh..
However, we couldn't manage to finish the second helping of BKT.. Everyone was too stuffed... But Will insisted not to waste to BKT soup cos according to him, BKT was something "holy" :P.. hahha... I know he was just kidding. Anyway, I managed to finish drinking the yummy BKT soup (I like to drink soup anyway)
I noticed Will had a "unique" habit, he poured the BKT soup into his chillies instead of the soya sauce. Klang BKT is nice & it's indeed something different from the BKT in Penang. Thanks, Will.

1U, KL
Went out with John on Sunday to 1 Utama.. This shopping mall is damn packed with people cos it's Sunday afternoon. Spending quite some time inside the parking lot searching for parking space. As usual, John said he will shoot me with his dunno watergun or airgun etc if he unable to find a parking space.. Well, at last he managed to find one.... :)
Had lunch in DOME. Then we walked inside 1 Utama for a while.. Visiting the MPH bookstore & ended up in the medical book section. John said he wanted to buy a medical dictionary to cheat back Will on Literati. (Well, say only mah...) At the end, I'm the one who end up buying the medical book, "Review of Medical Physiology" (22nd edition) by William F Ganong. I think everyone (if not everyone, also mostly..) who study medicine will know about this book, cos it's one of the recommended physiology text book.

I had a copy already at home, but it's the 16th edition, which I bought during my first year undergraduate time in 1993.. Well, so fast, now is the 22nd edition already.. My old book is still in "good shape".. Buy another one, and increase my book collection at home.. In fact, I have lotsa medical books already.

Hokkien vs Cantonese
Finally reach home, after a longggggg journey ... Home sweet home :)
My dog is so happy to see me :D
Well, Penangites speak in Hokkien while KL people converse in Cantonese. On my first day in KL, I talked in Hokkien to the cab driver. When he replied me in Cantonese, then only I realised I'm now in KL already & it's time to change dialect.. (I can speak Cantonese, Hokkien as well as Foochow..)
After staying for one week in KL, I sort of used to talk in Cantonese already. Today, on my way back home, My hubby stopped by the porridge stall to buy "Chee Cheong Chuk" for dinner. And I started ordering food in Cantonese. After I finish ordering, then only I realised I'm now back in Penang already :).. hahhaa.... Time to talk back in Hokkien liao..

Alliteration
I just read this tongue twister from DOBBS (posted by Dr Surey)
If one doctor doctors another doctor does the doctor who doctors the doctor doctor the doctor the way the doctor he is doctoring doctors? Or does the doctor doctor the way the doctor who doctors doctors?
When a doctor falls ill another doctor doctor's the doctor. Does the doctor doctoring the doctor doctor the doctor in his own way or does the doctor doctoring the doctor doctors the doctor in the doctor's way?
Hahaha... :D

Big Baby (aka macrosomia)
Guess what's the "biggest" newborn baby that I've ever see?
A 5.86 kg baby boy!!! This incident occured during one of my on calls in Maternity Hospital quite some time ago. This baby double the size of other newborn :) He is so chubby that he can't even open his eyes properly. He's so cute with his slit eyes appearance.
The Obstetrician suspected that the mother has big baby but didn't expect the baby to be so huge..(Estimated Birth Weight was around 4.2 to 4.5 kg) The mother was advised to undergo Caesarean section since there's high chance of shoulder dystocia if the baby is huge/large (macrosomia). The mother initially reluctant to go for Caesarean Section but she consented later on after weighing the risk & benefit that might be encountered..
Really thank God the mum opted & choose to undergo Caesarean Section.. cos even with Caesarean Section, the surgeon faced difficulty to deliver the baby out (cos the baby is really damn big !!!). Have to extend the surgical incision in the uterus & after some time, then only manage to deliver the baby with aids of forceps.. Can't imagine if the mother go for Vaginal Delivery --->it will really be hazardous. Shoulder Dystocia, is inevitable !!! The newborn baby really look like two months old kid, cos he is so huge :)
Mmmm.. I hope he won't grow up to be someone obese or overweight. Well, I have a morbidly obese patient not long ago.. He is only a 9-year-old boy with the body weight of 106 kg (morbid obesity) !!! That boy really has very good appetite & eat regular 6 heavy meals a day.. So many complications he encountered because of his morbid obesity..
The mother doesn't have Diabetes or Gestational diabetes (It's more common for poorly controlled diabetic mother to have macrosomia or huge baby) & she's not obese or big size.. (like the normal pregnant women with body weight around 70 kg).. However, she has history of macrosomia before with previous baby birth weight of 4 kg, & previous baby was delivered via Caesarean Section.

Packing..
Just finish packing my things.. Supposed to start packing earlier on, but I feel very tired.. So I have a short nap just now.. & wake up around 10 pm. Guess I still haven't fully "recovered" from the tiredness of my previous on call.. Now I feel better & more energetic after the short nap..
Ate two slices of bread for my late dinner..(Can consider that as supper already :P) In few hours time, I will depart to KL.. Haven't go to KL for almost a year already. My last visit there was in June last year.

Supper -> Satay 沙爹
My husband brought these satay(沙爹) back home yesterday night.. Bought on the way back home (from the "Sate Kajang Hj Samuri" corner in Restoran Jejantas Sg Buloh) from KL. Haven't eat supper(as well as satay) for so long already.. But these satay looks so tempting.. Hahhaa..What to do? eat loh.. Very tasty... Gave some to my dog as well.. My little doggy like it so much..
Dissection session
The first dissection session was held in the 5th week in our first year in medical faculty. (I was 19 years old + at that time..) What an experience - first time dealing or come in touch with corpses or cadavers. Were the bodies imported from outside? Is it from India? or somewhere else? I don't know ...
There were 14 cadavers altogether, each shared by 13 medical students; mostly were male cadavers. I didn't have any thrilled feelings... Friends outside described us as very brave... or "cold blood creatures"?. Well, I didn't really like the dissection session, which were held twice a week. Not that I was scared, but just that I couldn't stand the formalin used to conserve the bodies so that it won't decay or rotten as the same bodies were used for the anatomy classes for our entire first year. Why i hate the formalin? It causes tears to flow out from our eyes and furthermore, after the dissection session, most of us will feel the adverse effect like headache and dizziness/giddiness at night time. In other word, we had to go zzzzzz earlier.
Another thing that annoyed me was - people asking myself /my coursemates what we dissected during our medical student time... And they asked me (as well as my other friends) whether we dissected frogs/rats or etc etc animals... :D Wah !!! Why on earth did they think that we need to dissect this animals? Be more practical lah... Which part of anatomy in a frog resemble human's anatomy? Even though rat is mammal, there are differences. We did dissect these animals during our Biology practical session but that was in Form 6... Not during university !!! By the way, we are not going to be veterinarians or veterinary surgeons lah.. The only "way" to learn real human anatomy is to dissect a real human being ---> The answer is : to use cadaver/corpse for dissection... (Besides referring to human anatomy atlas etc etc... Real life, 3 dimensional "human models(cadavers)" is always better & clearer than the textbook or atlas pictures..

I purposely resize these pictures to "smaller size" cos some might be "scared" to see this sort of images.. :)

Another busy On Call Day..
Don't know why... My luck was damn bad & "sui" for my past few on calls... Yesterday has been busy the whole day & including night as well... Basically it's something like "running" around in the hospital.. My leg feel so "heavy" already by afternoon.. Really feel like lying down on the bed if possible but too bad, I can't :( (hahaha, if I lie down on the bed, I think I will fall asleep straight.. :P)&... getting few referrals at the same time.. pager beeping non stop... I feel so exhausted already by the evening.. and yet there'r still so many hours to go.. Receiving a phone call from Gasman as well... I can't answer him at that time cos I was busy intubating one case ... And after I attended to all my referrals, then only I call him back :P...
Sorrylah,Gasman, reply u so late, but I was indeed very busy... Btw, u say u are just informing me about the case, but at the end, I take over the case as well lah... but my ICU is full house, so we lodge the patient in the CCU, but still under our care.. She needs hemodialysis today.. Aiyoh.. half of my ICU is the patients that referred from your hospital lah.. One is the leptospirosis guy, (like PR in our ICU already, staying here for more than 50 days already...), one is the snake bite case and another one is the intra abdominal sepsis secondary to anastomotic leak... follow by this case... Btw, don't send any more cases over here again lah.. :P hahahaa....(just joking lah..) I even have one patient on differential lung ventilation in ICU as well.. hooked up to two ventilators on DLT (double lumen tube).
So fast, today is 1st of July already... Today have lotsa things to settle.. By the time I finish handling over the cases & doing morning rounds, it's past 10 am already.. Have to settle my on call claim & write two POMR (perioperative mortality review) reports.. both are neuro surgical cases..(Most of these mortality cases are neuro surgery cases...) If doing emergency craniotomy for evacuation of hematoma during on call, most of the time, will end up writing POMR :( Have to settle all these things cos I won't be around for the next whole week(going to KL)... We're given one week to write those reports..So, like it or not, I have to finish writing the reports by today..
By the time I reach home, it's 1 pm already.. I just rest for a short while, then go out again to settle my credit card bills & handphone bills.. and went back to the hospital again to settle my stuffs... Went to my father in law's place for dinner & fetch my little doggy back home :) I put my dog over there for the past two days.. Tonight I will go to bed early... :)
